Yes. Most of our first-time appointment forms are available online through this website. To open these forms to print, visit our Patient Forms page.

do you accept credit cards?

Yes, we do. We accept Visa, MasterCard and American Express for all or part of your treatment cost.

is financing available for treatments?

Easy monthly payments are available with approval through Chase Health Advance. If you would like to apply for financing, please ask either on the phone when scheduling your appointment or at the time of your appointment or visit one of the websites below:
